About Appointment and Outing Support
Getting to an appointment may require more than arranging a ride. The client may need help getting ready, someone to accompany them, or help returning to their routine afterward.
When offered and arranged in the care plan, Ms. Lewis’s Home Care can discuss transportation coordination and accompaniment for appointments and outings.
What this can include
- Help getting ready to leave home
- Transportation coordination, where arranged
- Accompaniment to appointments
- Trips to the pharmacy, bank, or stores
- Social and community outings
- Help settling back in at home
Specific tasks are based on the individual care plan and what the agency is equipped and authorized to provide.

Our approach
Tell us where, when, and what help is needed
Every outing is different. Tell us the destination, the schedule, and the assistance required so we can determine what can be arranged.
Caregivers can share relevant observations from the outing with the family through the proper channels.
