The Lewis Handoff

Care shouldn’t start over when a shift ends.

Different caregivers may sometimes be necessary. Disconnected care does not have to be.

The Lewis Handoff is our approach to helping relevant information move appropriately from one visit to the next.

Part of a connected system

One of five parts of the Lewis Human Intelligence System.

The Lewis Handoff is one of the five parts of the Lewis Human Intelligence System, guided by the Lewis Standard of Care. Look-Ahead identifies observations, Family Signal guides reporting, CareView informs authorized family contacts and the 30-Day Care Check reviews ongoing support.

The Lewis Handoff

Why the handoff matters

A client should not have to repeatedly explain their familiar routines. A family should not have to fill the same information gaps after every shift. A caregiver arriving for the next visit should have appropriate context for the support they are expected to provide.

The Lewis Handoff helps our team think about

  1. 01What happened
  2. 02What changed
  3. 03What matters
  4. 04What should carry forward

How it works

Today’s Visit → The Lewis Handoff → The Next Visit

  1. TODAY’S VISIT

Today’s visitThe caregiver follows the care plan, provides support, pays attention, and documents relevant information.

  2. THE LEWIS HANDOFF

Between visitsInformation that should be shared is communicated or escalated according to the care plan and agency procedures.

  3. THE NEXT VISIT

The next visitThe next caregiver receives appropriate context about the client’s routines, preferences, observations, concerns, and upcoming needs.

What may carry forward?

What may carry forward?

  • Daily routines and preferences
  • Assistance provided during a previous visit
  • Relevant observations
  • Concerns already reported
  • Family instructions
  • Schedule information
  • Details needed for the next visit

Only information appropriate to the client’s care and the caregiver’s role should be shared.

What families can expect

We work toward consistent care relationships because familiarity matters. When another caregiver is necessary, our goal is to help that person arrive informed. The Handoff supports continuity; it does not promise that the same caregiver or replacement coverage will always be available.
